Sleep Doc Direct PLUS Breathing allows patients with respiratory or sleep issues to make appointments directly via the Athena EHR web portal. Please click the link below to register for the portal and book  your appointment.

Please note:  We will be moving the patient portal from Charm EHR to Athena EHR. If you are a current patient and have previously registered with Charm EHR, please register with the new Athena EHR portal . The Charm EHR portal will not be functioning as of 10/1/2025.

APPOINTMENT TYPES: ​

Virtual Video Appointments: Offered completely online, virtual appointment are available to anyone within our service area with an adequate internet connection. You will conduct your visit using your smartphone or computer.

In-person Appointments: We now offer in-person appointments at our Durham Office, Sleep Doc Direct, PLUS Breathing. Our office is located in building 600 at Central Professional Park in North Durham, adjacent to Duke Regional Hospital. (2609 North Duke Street, ste 612, Bldg 600, Durham, NC 27704)

 

Which appointment type should I choose? Patients with sleep issues can be seen either virtually or in-person. Patients seen with predominantly respiratory issues (cough, trouble breathing during the day, lung nodules, or other lung disorders)  are seen in-person initially and then may have the option to be seen virtually during follow up visits.
