And Sleep Doc Direct, PLLC to provide: Sleep Sync+ Membership and Insights (Collectively referred to as “Service”)
Service
Client will be provided with monthly Insights, which will include the PAP report that has been reviewed by a healthcare professional member of Sleep Doc Direct. The Insights report may also include comments from the healthcare professional regarding their findings upon review of the report.
The information contained in this Terms of Use is a to provide a general overview of Service. In signing this document you acknowledge that you have had ample time to read and understand the Sleep Doc Direct Master Services Agreement Policy and agree to abide by it.
COST
Membership Fees:
6 Months: $99.00
Month-to-Month: $19.95
Billing
• Month-to-Month: Your Monthly Membership Fee will be billed to the account below on or around the first Wednesday of each month.
• 12 Month membership: Your Membership will expire 12 months from the date of your first report. The membership will then be renewed at the prevailing rate as published on the Sleep Doc Direct website. You will receive an email at the time of your final Insight report informing you of the new rate and your option to continue or discontinue.
Cancellation
You may cancel Service at any time. To cancel Service and stop the billing of the Monthly Membership Fee, you must provide us written or electronic notification via one of the methods below.

Email: support@sleepdocdirect.com. Upon email notification you must call to Sleep Doc Direct at 919-951-7778 to ensure that the email communication was received.

Refund Policy
Month-to-month Membership: Months are billed at the time Service has been rendered after which refund will not be provided.
Semi/Annual membership: Membership is billed as a 50% registration fee plus monthly installments.You will be refunded at the rate of 5$ for unused months for semi/annual memberships. Each monthly Insight report is considered as one month of service. Months are not prorated. Months begin and end with the delivery of the Insight Report.

Please note that Service is not intended to diagnose, treat, cure, or prevent any disease or medical condition. The Insights report can not replace a visit with a healthcare professional. The Insights report may provide general insights into the clients pap data. This data has limitations as it can not replace an evaluation by a healthcare professional. The data can not elucidate all
forms of apnea and may by limited by errors as well as other factors having to do the technology, the client, the interface used by the client. The information and guidance in the Service are there for informational purposes only and can not replace the services of health professionals. You should always consult a physician before making any changes to your sleep or activity based on information or guidance of the Service or if you have any questions regarding a medical condition. Never disregard professional medical advice or delay in seeking it because of something you have read on or through the Service. We are not responsible for any health problems that may result from information or guidance you learn about through the Service. If you make any change to your sleep or activity or care regimen based on the Service, you agree that you do so fully at your own risk. You agree to defend, indemnify and hold Sleep Doc Direct and its partners, affiliates, service providers, licensors, officers, directors, employees and agents harmless from and against any claims, actions or demands, including but not limited to reasonable legal and accounting fees, alleging or resulting from your violation of these Terms of Use.
